The form must function exactly the same as that link above with the same required fields. User must be able to submit the form and see a confirmation (just like attached). The submitted form entries must be submitted to a email address I will provide.

i am looking for a module that users can transfer domains between each other.

Details of module:
When an user click to domain transfer button, module ask him/her to type in the email of the person who the domain will be transferred to.

Fix Tomatocart 1.1.5 Customer Email Module

I have made an email address and account just for the online shop I’m making. I cannot get TomatoCart to setup the account in the customer email module on the Admin side. I have tried setting up other accounts as well, and still the same issue. When I get all the info entered and click ok, it says “Please waiting…” and then it just goes back to the window before I clicked ok and nothing’s been added as an account.
