Need Simple PSD to Wordpress

I have a simple PSD design ( see attachment ) I need in Wordpress.

Home page will have slideshow
3 Images easy to manage ( maybe use widget )
footer easy to manage.

Subpage no need for slideshow, only static image I will provide.

Since this is a very simple design, and I only need the home page and subpage set up, I need this done as soon as possible. If you want to use a template and modify it to look like my PSD, thats fine. need this done quick.

Modify A Simple Survey Wordpress Plugin

Need an existing wordpress survey plugin to be more customized.

1. Need it to allow for html code from my email responder software. So when they take the quiz they are opting into my email subscriber list.

2. Make it possible to have 3-5 possible answer selections per question

IE 8/9 fix for dating site

I have a gay dating site (cakePHP/Mysql) which has a IE 8/9 issue. Members cannot login when using windows IE 8/9. When members enter the login and password nothing happens after they click on the submit button.

This site is hosted on Dotcloud. THERE IS NO FTP OR CPANEL ACCESS, ACCESS IS ONLY VIA GITHUB.

Also, I have two style sheets that allow me to override the two main ones.

When I modify the styles they do not take effect. This will have to be looked at.
