PhP Contact form - PDF submission

I have a pdf form which I want to convert to a contact form. This form captures clients personal information such as name, date of birth, etc.

After the client hits the sign up button I need the data to be reformatted into a pdf file (we have a sample) and be sent to an email.

Update an existing HTML/JS/PHP contact form.

There is a simple HTML form that consists of 10-20 fields.

3 fields need to become conditional, and their availability for user depends on the value of a different field.

Also, we need to add an alert (message box) for certain fields.

Contact Registration Form Template

Contact form 7 Javascript validation problem in IE8

I need assistance from an expert to fix an issue with the form validation. The contact form works fine in IE9, Chrome and firefox but is not working properly in IE8.
